Medical Social Worker

A Medical Social Worker supports not only the patient, but the family and caregiver with financial and legal concerns (including Advanced Directives, Do Not Resuscitate, Durable Power of Attorney, Living Will, and long-term planning.) They provide social and emotional counseling, and also identify and provide community resources.
Danny's Home Heathcare employs medical social workers who specialize in providing assistance with obtaining necessary community resources. This can include anything from “Meals on Wheels” to counseling. Some of the resources that we can help connect you with include:
- Help with areas of daily living (“ADLs”)
- Providing in-roads to senior housing, assisted living facilities, and skilled nursing facilities
- Caregiver assistance and respite services
- Companionship
- Help with chores such as shopping, cooking, and cleaning
- Health and wellness programs
- Educational opportunities
- Social services
- Adult day care
- Connections with associations and agencies that are condition-specific (such as the American Alzheimer’s Association, Veteran’s Administration, American Cancer Society, etc.)
